Self- Management Workshop Series

Chronic Disease Self Management Program (CDSMP) is a 6-week group workshop series developed for people with at least one chronic health condition, which may include arthritis.

Informational session on June 3, 2024, 1:00 pm - 2:00 pm

Classes begin June 10, 2024!

Classes will be held at the Forest Hills Heights Community Center: 700 South Mangum St. Durham, NC.

Topics include:

• Falls prevention
• Healthy eating
• Working more effectively with healthcare providers
• Action-planning, problem-solving, decision-making
